What are the hours of operation? Monday through Friday from 8:00 a.m. to 5:00 p.m. Walk-ins from 8:00 a.m. to 11:00 a.m. and 1:00 p.m. to 4:00 p.m. Where is ClinicCARE located? ClinicCARE is directly behind Sumner Regional Medical Center in Gallatin. We are located in Suite 6B of the Medical Mall Building in Gallatin, 575 East Bledsoe Street. From the Square in Gallatin, head east on East Main Street and Hartsville Pike until you pass the new multi-story parking garage at Sumner Regional Medical Center. Immediately after passing the parking garage, turn right onto Steam Plant Road. East Bledsoe street is just behind the hospital. Turn right onto East Bledsoe Street. The Medical Mall of Gallatin will be about 100 yards ahead on your left. How can ClinicCARE be reached? Simply call (615) 451-6296 during office hours to schedule an appointment or to get additional information. After hours, please leave a message. What does it cost for a patient at ClinicCARE? The cost of an office visit is $35.00. The only additional costs, for example, would be if you received an injection (it will be the cost of the medicine only), lab tests, pregnancy test, urine testing, etc. (Lab services will be provided at cost at the time of service.) What is the cost if I just want to get a flu shot? The total cost is $25.00. What services are provided through ClinicCARE? The clinic functions like any other primary care physician's office setting. What forms of payment does ClinicCARE accept? We accept cash, credit cards and debit cards. We can also bill Medicare, TennCare, AmeriChoice, AmeriGroup, CoverTN, and TriCare directly. If I have insurance with another company, can I still visit ClinicCARE? Yes. We will give you paperwork that you can submit to your insurance company. They will usually reimburse you for all or part of what you paid to ClinicCARE. Do I have to live in Sumner County to visit ClinicCARE? No. All are welcome to use our services. Do I have to live in Tennessee to visit ClinicCARE? No. All are welcome to use our services. How is ClinicCARE structured in staffing? The clinic has a Certified Physician Assistant, Medical Assistant, and Support Staff. A physician preceptor oversees the clinic's medical services and will be able to admit the patient to the hospital if necessary. |
